Case Management
Mission
The mission of Bremer County Case Management
is to provide assessment, referral, coordination and monitoring
of support services for individuals with disabilities to achieve
their choices and desired goals.
Who Qualifies
Persons who have mental disabilities
and are in need of support services to live and work in the community
of their choice. Persons must be eligible for T-XIX benefits or
Bremer County funding for services.
