About the Program
The Bremer County Health Department's provides immunizations to all age groups for protection against vaccine preventable diseases.  The BCHD also contracts with Iowa Health & Human Services to provide federally-funded vaccines through the Vaccine For Children Program.

The Bremer County Health Department is enrolled in Iowa Immunization Registry System (IRIS), which is a secure statewide database of immunization histories.  Vaccines available through the BCHD are those recommended by the Advisory Committee on Immunization Practices (ACIP).

Making an Appointment
Call the BCHD at (319) 352-0082 to check on the availability of vaccines. An appointment can be scheduled at our facility after confirming vaccines needed are in stock.  Please have the following information available during the call and at your appointment:

  • Immunization record
  • Insurance card
  • Date of birth or social security number
Children birth through 18 years of age qualify for the Vaccines for Children Program if they fall into one of the following eligibility criteria:
  • Medicaid-eligible
  • American Indian/Alaskan Native
  • Uninsured
  • Underinsured
Iowa Immunization Requirements
Iowa Code, Chapter 139a.8(6) and Iowa Administrative Code, 641-7.7(139) outline the immunization requirement for students attending licensed childcare centers and elementary or secondary schools.  Students shall have received the required immunizations and submit an Iowa Certificate of Immunization or have a valid Certificate of Immunization Exemption-Medical or -Religious, or Provisional Certificate of Immunization.


Requesting Immunization Records or Replacing Lost COVID-19 Vaccine Cards
Complete and submit an immunization record request form to the Bremer County Health Department by following the instructions within the form.  Please allow 3-5 working days to process the record request.


Flu and COVID-19 Vaccines for 2023-2024
The BCHD flu vaccine is $30.00 per vaccine cash or check only.  We are unable to bill insurance companies for the flu vaccine. COVID-19 vaccines are free of charge for individuals who qualify. To see if you qualify for free COVID-19 vaccines, click here.
