Building a Healthier Bremer County


Home Care Aide Program- Offers personal care and homemaker services:

Personal Care services may include: Bathing, shaving, dressing, range of motion exercises, and meal preparation.

Homemaker services may include: Light housekeeping, meal planning and preparation, shopping for food and essentials, and laundry.

Skilled Nursing Program- Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides services under your physician's orders such as: wound care, catheter or colostomy, blood draws, assistance with medication management, post-hospitalization follow-up, medication injections and influenza immunization, etc.

Rehabilitative Services Program- Physical therapy, occupational therapy, speech therapy, and medical social worker.

Loan Closet Program- Durable home care equipment for borrow.

The Bremer County Health Department is a Medicare Certified Home Health Agency.  Skilled nursing at home is more affordable, and often with better results and shorter recovery time than institutionalized options.

The Bremer County Health Department has experienced home health professionals available for your needs 24 hours a day, 365 days per year.


Community Assessment- Assessment of Bremer County communities to determine needs and assist in program planning.

Communicable Disease Program-  Education to the public concerning communicable disease and prompt follow-up to communicable disease outbreaks.

**NEW** Diabetes Prevention Program - A free structured 12 month weight-loss program provided by a trained lifestyle coach.  Participants must be 18 years of age or older and not already be diagnosed with diabetes.  For more information and to enroll in the Prevent T2 class, click here

Immunization Program- Recommended immunizations for both children and adults at a reasonable cost.  Call us at 319-352-0082 to schedule an appointment to receive your immunizations.

Vaccines for Children Program- Recommended immunizations for children through 18 years of age who are uninsured, underinsured, American Indian or Alaska Native, or Medicaid eligible.  Call us at 319-352-0082 to schedule an appointment or visit us the third Tuesday of every month from 8:45 am – 5:15 the WIC clinic at Redeemer Lutheran Church-2001 W. Bremer Avenue in Waverly.

Foot Clinics- Provide accessible, affordable foot health care to community residents aged 55+ years of age and/or significantly disable who have difficulty with their own foot care due to vision, agility, or whatever detriments may preclude regular foot care.  Foot clinics are provided by a skilled nurse.  Foot clinics are held the 1st Wednesday of every month at the Bremer County Health Department.  Call 319-352-0082 to schedule an appointment.  Recommended minimum donation of $25.

Referrals are accepted from anyone and anyone who is eligible if the need exists.  There is no cost for the confidential needs assessment.  Services are determined by the participant with a Bremer County Health Department Representative.
